| ARRA Reporting |
In addition to all 1512 Reporting Requirements the following apply: The reporting requirements are set forth in the Attachment 2 of Funding Opportunity Announcement No. DE–FOA–0000051. Financial and progress reports will be used to adhere to the transparency and oversight requirements detailed in the Recovery Act. The reporting requirements primarily consist of existing reports that are regularly submitted by DHCR as a part of the Weatherization Assistance Program management. In addition to the existing reports listed below, a new report outlined in #4B below is required. DHCR will supplement its existing database system with a spreadsheet to collect the information relating to the new report requirements. It should be noted that all reporting will be specific to funding provided under ARRA. 1. Management Reporting A. Progress Report – due quarterly within 30 days after reporting period and a final report due 90 calendar days after expiration or termination of award. Must include concise narrative assessment of the status of work. (This is an existing report which is also referenced in # 4B, below). 2. Financial Reporting A. SF –425, Federal Financial Report due quarterly and final. (This is an existing report that DOE requires and the information is reported through the DOE WinSaga system). 3. Closeout Reports A. Property Certification due at final (An existing report that applies to equipment purchased with federal funds but not yet transferred to subcontractors). 4. Other Reporting A. Annual Indirect Cost Proposal Report – due annually (An existing report that OSC handles for all State agencies). B. WAP Report (DOE F 540.3) due quarterly (see #1A, above). C. Annual Training, Technical Assistance, Monitoring and Leveraging Report (Existing DOE Form 540.4) due annually. D. ARRA Performance Progress Report due quarterly 10 days after end of reporting period and no later than 180 days of enactment of ARRA (August 2009); This is a new report developed for ARRA containing the following information: –total amount of ARRA received from that Agency –amount of ARRA covered funds received that were expended or obligated to project or activities –a detailed list of all project or activities for which ARRA funds were expended or obligated including: name of project, description of project, evaluation of the completion status of project, estimate of jobs created and retained by project, infrastructure investments made by State and local governments, information on subcontracts or subgrants awarded by recipient to include date element required to comply with the Federal Accountability and Transparency Act of 2006(Pub. L. 109–282); DHCR has been verbally advised by DOE staff that for WAP purposes, “Project” means each individual contract between DHCR and a subgrantee. This information is all also included in the WAP quarterly report (#4B above). |

The U.S. Department of Energy distributes the funds by formula to States who award the funds to qualified weatherization providers. These providers accept applications, determine eligibility, conduct energy audits and perform weatherization activities in the homes of low income households. |

Qualified nonprofits receive grants from state agency. Low income households may apply to have their home weatherized by contacting the State funded Weatherization provider in their county.
